UNIIKS Care Assisted Living is seeking a compassionate, dependable, and experienced Registered Nurse (RN) to join our growing team. As an RN, you will play a vital role in ensuring our residents receive exceptional nursing care while maintaining compliance with state regulations and promoting their health, independence, and quality of life. The RN Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Responsibilities

  • • Completes an initial assessment of client and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • • Regularly re-evaluates client nursing needs.
  • • Initiates the plan of care and makes necessary revisions as client status and needs change.
  • • Uses health assessment data to determine nursing diagnosis.
  • • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the client and the family in the planning process.
  • • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act.
  • • Counsels the client and family in meeting nursing and related needs.
  • • Provides health care instructions to the client as appropriate per assessment and plan of care.
  • • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client.
  • • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
  • • Lead by example in establishing rapport with clients, clinicians, partners, and the community.
  • • Ensure positive/professional communication within the team
  • • Customer service resolution; escalate concerns
  • • Prepares clinical notes and updates the primary physician when necessary and at least every 60 days.
  • • Communicates with the physician regarding the client’s needs and reports any changes in the client’s condition; obtains/receives physician’s orders as required.
  • • Communicates with community health related persons to coordinate the care plan.
  • • Timely completion of all documentation as it relates to the care plan by 10am the next day
  • • Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse
  • • Completes orientation and annual training requirements as assigned
  • • Participates in UNIIKS’s Event Management process including initiating reports, escalation of events and investigations
  • • Promotes safety in the workplace through real time coaching and participation in safety awareness training
  • • Instructs, supervises, and evaluates home health aide care provided every two (2) weeks
  • • Manages client supplies assuring appropriate DME provider is utilized.
  • • Schedules and keeps member appointments
  • • Organizes travel efficiently and documents mileage accurately within EMR
  • • Other duties as assigned

Required qualifications

  • • One to two years of recent acute care experience in an institutional setting. One year of home health care experience preferred.
  • • Must pass background check
  • • Experience in customer service/direct member contact field preferred
  • • Experience supervising caregivers
  • • Graduate of an accredited school of nursing
  • • Current Minnesota Registered Nurse (RN) or Licensed Practical Nurse (LPN) License
  • • Must pass a MVR check based on UNIIKS company policy
  • • Must be able to relate professionally, positively, and cooperatively with residents, resident’s families, and employees
  • • In accordance with CMS Vaccine Mandate, candidate must provide proof of Covid-19 Vaccine or submit documentation for exemption

Preferred qualifications

  • • Experience working in Assisted Living or Memory Care.
  • • Knowledge of Minnesota Assisted Living regulations.
  • • Experience supervising caregivers or nursing staff.
  • • Experience with medication management and resident care planning.

Physical requirements

To perform the essential functions of this position, the employee must be able to: • Experience working in Assisted Living or Memory Care. • Knowledge of Minnesota Assisted Living regulations. • Experience supervising caregivers or nursing staff. • Experience with medication management and resident care planning. • Stand and walk for extended periods throughout the workday. • Frequently bend, stoop, kneel, crouch, reach, and twist while providing resident care. • Safely lift, transfer, push, or pull up to 50 pounds independently and assist with heavier transfers using proper body mechanics and mechanical lifting equipment. • Assist residents with ambulation, repositioning, and activities of daily living as needed. • Respond quickly and appropriately to medical emergencies. • Maintain the physical stamina to move throughout the facility and between resident rooms. • Possess sufficient manual dexterity to administer medications, perform nursing procedures, and operate medical equipment. • Demonstrate visual acuity to observe residents, read medication labels, review documentation, and monitor changes in residents' conditions. • Demonstrate hearing and verbal communication skills sufficient to communicate effectively with residents, families, physicians, and team members in person and by telephone. • Work in an environment with frequent exposure to infectious diseases, bodily fluids, cleaning chemicals, and other healthcare-related hazards while using appropriate personal protective equipment (PPE). • Meet all health screening, immunization, and tuberculosis (TB) testing requirements in accordance with state regulations and company policies. • Reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions of the position.
